Requirements

• Minimum of 2 years recent RN experience preferred in medical-surgical, critical care, emergency, step-down, or other setting caring for medically complex adult patients. Experience within the past 5 years preferred. Experience in primary care, ambulatory care, or a comparable outpatient setting managing acute and chronic patient care needs. Experience with telephone and face-to-face triage, including symptom assessment, identification of red flags, determination of appropriate level of care, and timely escalation when indicated. Experience caring for patients with common chronic conditions such as diabetes, hypertension, cardiovascular disease, COPD, and other complex medical conditions. Experience coordinating care and follow-up across primary care, specialty care, pharmacy, social work, inpatient services, and other disciplines. Experience providing education related to medications, disease management, preventive care, treatment plans, and self-management. Current experience with medication administration, immunizations/injections, point-of-care testing, bladder scanning/post-void residuals, ear irrigation, wound care, and other ambulatory nursing procedures. U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
